Sponsorship Request Form

To submit a Montcalm Community College Sponsorship request, complete the information required in all fields of the request form below. You will be notified when your request has been received, and again when a decision has been made regarding your request. Verbal commitments from faculty and staff do not constitute a commitment from MCC.

Please describe your project, including what it is, and who will benefit.

If there is an organized event associated with this project (such as a competition, conference, etc.), please explain what the event is, how participants will benefit from participating in the event, and provide the date and location of the event.

What are your specific funding needs (i.e. supplies, conference expense, etc.). Why are you requesting funding from MCC?

What is the amount of your request? What is the breakdown of funding (i.e. specific costs for each element, such as travel, supplies, t-shirts, etc.)

Please provide the names of individual students who will participate in the project. In the case that this is a competition or similar event where the names are not available upon submitting your request, you will be required to provide this information before funds are disbursed. Failure to do so will result in forfeiture of any funding from MCC.
